Course Cancellation Request Form

The purpose of this form is for you to notify us that you wish to cancel your course.

When you signed up for the course, you agreed to the following terms and conditions. Based on these terms, a decision will be made on your refund request:

1. A learner may withdraw from a course after registering by submitting the Course Cancellation Request Form. This form must be submitted within 14 days of the original purchase date.

2. The right to cancel during the cancellation period is lost if:

   • Forus Training begins providing training at your request before the 14-day cancellation period has passed.
   • The course is already open for you to begin.
   • The course begins within the 14-day window, as the service is considered rendered.
   • Course materials or Learning Management System (LMS) access details have been issued.

3. In the event of a cancellation, a non-refundable administration fee of €100 applies.

4. A two-week grace period will be provided if instalment payments cannot be collected. After that, a fee of €10.50 will apply for each missed instalment.

5. If instalment payments are not made, your access to the course and tuition will be suspended unless satisfactory payment arrangements are made.

6. In case of medical reasons, you may request to put your course on hold for a maximum of six months, subject to the submission of a valid medical certificate and approval by Forus Training.
